TL;DR: You can now serve Polyform forms from your own domain (e.g., forms.yourcompany.com). Set up takes a DNS record and a verification step. Multiple domains per team are supported.
#Why this matters
Default form URLs work, but they don't build trust. When you send a form to a customer, partner, or job applicant, the URL is the first thing they see. forms.yourcompany.com/feedback tells respondents they're in the right place. A third-party URL makes them pause.
Custom domains also matter for brand consistency. If your marketing site, docs, and app all live on your domain, the form should too.
#How it works
- Add your domain in the Polyform dashboard under team settings
- Add a DNS record — we provide the exact CNAME or A record to add with your DNS provider
- Verify — Polyform checks the DNS record and activates your domain
- Share forms — your forms are now accessible at
yourdomain.com/form-slug
The setup takes a few minutes, most of which is waiting for DNS propagation.
#Multiple domains
Teams can add more than one custom domain. This is useful if you run forms for different brands, products, or regions and want each to use its own URL.
#What about existing links?
Your default Polyform URLs continue to work. Custom domains are additive — they give you an additional way to share forms, not a replacement. When sharing a form, the dashboard now shows your custom domain URL alongside the default one.

Custom domains are available on the Pro plan. Set them up in your team settings under the domains section.
