Every form in Polyform can be fully customized to match your brand. Switch to the Design tab in the form builder to access theming options.

#Colors

Customize a full color palette for your form:

Primary / Primary foreground — Buttons, links, and accent elements
Secondary / Secondary foreground — Supporting UI elements
Accent / Accent foreground — Highlighted elements
Background / Foreground — Page background and main text
Card — Card and container backgrounds
Muted / Muted foreground — Subtle backgrounds and secondary text
Border — General borders
Field border / Field background — Input field styling
Ring — Focus ring color

#Fonts

Choose from a selection of fonts:

Title font — Used for form titles
Description font — Used for descriptions and body text
Question title font — Used for question labels

#Border Radius

Adjust the border radius to control how rounded your form elements appear.

#Cover Image

Add a cover image to the top of your form:

Upload an image
Set separate heights for mobile and desktop
Adjust the focal point for cropping

#Branding Pro

All forms include Polyform branding by default. With the Pro plan you can replace it with your own custom branding:

Logo — Upload your logo to display on the form
Custom text — Customize the branding text (e.g., "Powered by")
Link URL — Where the branding links to

#Theme Templates

Choose from pre-built themes to get started quickly, then customize from there.

#Polly AI Pro

You can ask Polly to adjust your theme using natural language. For example, ask for "dark theme with purple accents" or "warm earth tones" and Polly will update your colors to match.
