Create your first form in under a minute.

#Create an Account

  1. Go to app.polyform.to and sign up with Google, GitHub, or email
  2. Follow the onboarding — you'll create a team and your first form as part of the setup

#Build Your Form

Once you're in the builder:

  1. Add questions — Use the + button to add questions. Choose from text, choice, rating, and many more question types.
  2. Customize the theme — Open the Design panel to customize colors, fonts, and branding to match your brand.
  3. Set up logic — Optionally add conditional logic to create dynamic flows.
  4. Publish and share — Click Publish to make your form live and copy the share link.
